MDS & Case Manager: Care Plan Leader

Roseville Healthcare & Wellness Center is seeking an experienced MDS/Case Manager to coordinate the RAI process and manage interdisciplinary care plans. The ideal candidate should have a Nursing Degree and a current nursing license, along with outstanding knowledge of the Medicare process. Excellent communication skills and a solid understanding of regulations are essential. Responsibilities include liaising between patients and providers, managing care plans, and ensuring timely documentation.
