Responsibilities
- Conduct PT evaluations and establish individualized treatment plans for residents
- Implement evidence-based interventions to improve mobility, strength, balance, gait, and functional independence
- Reassess progress, adjust goals, and modify treatment in collaboration with the interdisciplinary care team
- Document assessments, plans of care, progress notes, and discharge planning in the medical record
- Educate residents and families on exercises, safety, and fall prevention; collaborate with the interdisciplinary team

Requirements
- Active physical therapy license in Washington state or eligibility for licensure
- Doctor of Physical Therapy (DPT) degree from an accredited program
- 1 year of physical therapy experience in a skilled nursing or geriatric rehab setting preferred
- Ability to work in a travel assignment and maintain flexibility for a 5x8 day shift
- Strong communication and documentation skills; ability to collaborate with a multidisciplinary team
