Nurse Case Manager - PACE Program (Day Shift)
Ascension Wisconsin Corporate Office
Ascension's St. Vincent PACE program in Indianapolis is seeking a full-time Day RN to provide direct nursing care to participants in the PACE setting. You will perform acute triage, blood draws, and wound care, and participate in the development, implementation, and evaluation of the interdisciplinary team (IDT) plan of care. Some travel to participants' residences or care facilities may be required. Licensure as an RN and BLS within 30 days are required. #J-18808-Ljbffr Ascension
