Minimum Requirements

Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Washington State Physical Therapist license is required to start the assignment. BLS/CPR certification required. Home health experience required. Experience with SOC, evaluations, ROC, and recertifications preferred. Experience with HCHB electronic medical record preferred. Ability to cover patients within a 50 mile radius of the primary office location. Ability to complete weekly 40 hours as scheduled. Business casual dress required.

Facility type: Home Health. Shift name: Days. Shift details: Days (schedule noted as Days 8:00 am - 4:30 pm in client notes). Weekly guaranteed hours: 40. On call/weekend requirement: No. Coverage area: 50 mile radius; mileage reimbursed between visits from first to last patient. Primary responsibilities: Start of Care (SOC), evaluations, return of care (ROC), recertifications, and subsequent visits.
