Remote Field Care Coordinator RN - Chronic Care Navigator
Registered Nurse
UnitedHealth Group is seeking a Care Coordinator to serve as the primary manager for a panel of members with chronic and complex health needs. You will support a person-centered approach and coordinate with the care team to improve outcomes and access to needed services.
The role requires strong organizational and communication skills, the ability to multitask in a fast-paced environment, and a commitment to equity in care. Remote work within eligible Idaho markets is possible.
