ICL is seeking an Occupational Therapist to join its Health Connect Program within Supported Housing in Brooklyn, NY. You will conduct health assessments, develop individualized care plans, and support clients in achieving recovery goals through trauma‑informed, person‑centered care approaches. Work closely with a multidisciplinary team to promote independence, community integration, and access to healthcare resources in supportive housing settings.
