Minimum one year’s experience Current state license as a Registered Physical Therapist / PT or be in the process of obtaining it Current CPR certification. This includes providing direct patient care, assessing medical condition, functional abilities, limitations and restrictions, and determining potential for rehabilitation. The PT develops and implements a treatment program with specific goals tailored to the patient’s capacity and tolerance under the direction of a physician, sets realistic goals, documents patient progress, and educates patients and families on appropriate therapy methods. Develop a plan of care for each patient. Provide skilled physical therapy services and interventions in accordance with physician orders. Assess patient needs, plan for, evaluate and modify care to meet goals of physical therapy interventions. Collaborate with all disciplines to plan and evaluate team goals for each patient.
