Substance Use Disorder (SUD) Case Manager - San Francisco, CA (Mission District)

Job Type: Full-Time, Permanent (after 6-month introductory period)

About the Role:

We are seeking a compassionate and motivated Substance Use Disorder (SUD) Case Manager to join a community-based organization serving youth and families in the Mission District of San Francisco. The SUD Case Manager will provide prevention and early intervention services for at-risk or violence-involved youth, focusing on culturally responsive healing, community engagement, and long-term wellness.

This role is part of the Roadmap to Peace (RTP) initiative - a collaborative program aimed at improving economic, health, and safety outcomes for Latino/a youth ages 13-24.

Responsibilities:

• Conduct outreach to engage youth in SUD prevention and early intervention programs.

• Perform SUD screenings and provide brief interventions.

• Develop and facilitate at least six SUD harm reduction workshops annually.

• Provide case management for up to 8 youth per year, including intakes, assessments, and care coordination.

• Connect clients to community resources, including education, employment, and treatment programs.

• Collaborate with schools, probation officers, agency staff, and partner organizations.

• Provide court advocacy and support letters as needed.

• Maintain accurate documentation, reports, and service logs.

• Participate in community-building activities and network meetings (some evenings and weekends required).

Qualifications:

• Bilingual (Spanish/English) required.

• Bachelor's degree in Social Sciences or related field (or equivalent experience).

• Minimum 2 years of experience in case management and substance use disorder services.

• Proficiency in SUD screenings, brief interventions, and case plan development.

• Experience facilitating harm reduction workshops.

• Experience working with at-risk or justice-involved youth preferred.

• Willingness to work flexible hours (morning, evening, and weekend availability).

• If in recovery, must be clean and sober for at least 2 years.

Job Requirements:

• Verification of degree(s) and required licenses prior to start.

• TB clearance prior to hire and annually thereafter.

• Completion of all annual training requirements.

• Position covered under SEIU 1021 collective bargaining agreement.
