Registered Nurse - Surgical and Specialty Care Float

In this float role, you collaborate with the interprofessional team, serve as a trusted liaison with community providers, guide assessments and transitions, and advance evidence-based practices, quality improvement, privacy, and exceptional service-ensuring care that honors every Veteran's needs, values, and voice. Major Duties Include, But Are Not Limited To:

• Plans, integrates, implements, modifies, and evaluates the effectiveness of the Veteran's care.
• Serves as a liaison to internal and external providers, managing care and the seamless transition to and from the community.
• Follows the standardized care coordination model, incorporating case management principles and processes for referral, authorization, and secure medical documentation exchange.
• Provides care and counseling; promotes and maintains health; prevents illness and injury using the nursing process; executes medical regimens including administering medications and treatments.
• Actively participates in unit/program level quality improvement processes and initiatives and customer service programs.
• Demonstrates effective communication to collaborate and co-manage care with the multidisciplinary team in a cost-effective manner.
• Provides initial and ongoing assessment, including telephone care assessment/triage, and identifies resources and interventions for emergent, urgent, or non-urgent needs.
• Implements patient education plans; supports and enhances patient self-management and determination; advocates for patients, families, and caregivers.
• Assumes responsibility for the seamless transfer of the patient to receiving facilities and facilitates communication and coordination among the health care team.
• Protects printed and electronic files; safeguards Veteran privacy; uses the Electronic Health Record (EHR) and Bar-Coded Medication Administration (BCMA) per policy; reports privacy and security incidents.

Pay: Competitive salary, regular salary increases, potential for performance awards
