Contract | Days | Monday–Friday (flexible shifted schedules such as Tuesday–Saturday or Sunday–Thursday may be available)

Key Responsibilities

• Administer and perform therapeutic interventions; assess clients' needs and develop individualized plans of care
• Cover SNF and Home Health settings across multiple locations, with travel between sites; mileage and drive time between locations is compensated
• Document observations, services provided, and clinical records using Net Health and HCHB software
• Notify physicians of changes in client status and provide direct consultation with physicians and interdisciplinary team members
• Train clients, family members, and staff on therapeutic interventions and equipment
• Maintain client dignity, privacy, and confidentiality while adhering to infection control and safety standards

Required Qualifications

• Active Nebraska SLP state license/certification required
• Completion of an accredited Speech-Language Pathology program
• Must be at least 18 years of age
• Ability to meet SNF productivity expectation of 82% (evaluations, group, and concurrent time counted in full)
• Ability to meet Home Health productivity expectation of 90% (inclusive of drive time and documentation)
• Must live at least 80 miles from the facility to be eligible for standard rates (local rates apply within 80 miles)

Preferred Experience / Skills

• Prior experience in a rehabilitation or geriatrics setting
• Familiarity with Net Health and/or HCHB documentation platforms
• Experience working across SNF and Home Health care settings
• New graduates are welcome — onsite therapy supervision and clinical support are available
