Discharge & Care Coordination Social Worker

McLaren Health Care in Port Huron, MI is seeking a social worker to assess and intervene to help patients and families resolve social, economic and discharge planning needs. You will coordinate post-hospital care, connect to community resources, and work with an interdisciplinary team to ensure safe transitions.
