- Conduct patient evaluations in inpatient or outpatient environments and develop personalized treatment plans
- Deliver physical therapy interventions that emphasize mobility, strength, stability, and functionality
- Team up with nurses, physicians, case managers, and additional rehabilitation experts to promote optimal results
- Inform patients and families about therapeutic objectives, discharge strategies, and at-home exercise regimens
- Maintain meticulous documentation of evaluations, treatments, and progress, following facility and regulatory protocols
- Engage in care team discussions and interdisciplinary meetings to ensure seamless patient care
- Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
- Active state licensure as a Physical Therapist
- CPR certification is preferred
- Additional specialized certifications (for example, in neurology or geriatrics) are advantageous but not mandatory
- Prior experience is highly desirable
- Ability to work with patients facing multifaceted medical and mobility challenges
- Exceptional communication and teamwork abilities
- Familiarity with digital charting and clinical documentation processes
