Ambulatory Registered Nurse

As an Ambulatory Registered Nurse in our Diabetes & Endocrinology practice, you will play a vital role in delivering specialized care to patients managing chronic endocrine conditions.

You will collaborate closely with endocrinologists, advanced practice providers, and care teams to provide evidence-based, patient-centered care, including in-office care, telehealth services, and chronic disease management support.

Deliver comprehensive nursing care using the nursing process (assessment, planning, intervention, evaluation) with a focus on chronic disease management.

Support patients with diabetes care, including blood glucose monitoring, medication adherence, and lifestyle management.

Educate patients on:

• Insulin administration and injectable therapies
• Continuous glucose monitors (CGMs) and insulin pumps
• Nutrition, weight management, and lifestyle modifications

Collaborate with providers to develop and implement individualized endocrine care plans.

Administer medications, vaccines, and perform point-of-care testing (e.g., A1C, glucose checks).

Provide telephone triage and telehealth support, addressing patient questions and symptom management.

Document care accurately in the EMR and assist with pre-visit planning.

Monitor and follow up on lab results, medication adjustments, and care plan adherence.

Serve as a resource and mentor to clinical staff; assist with onboarding and education.

Respond to urgent clinical concerns in alignment with established protocols.

Associate of Science in Nursing (ASN) required; Bachelor of Science in Nursing (BSN) preferred.

Current Registered Nurse license in NH (or NLC state/MA) and BLS certification required.

1+ year of professional nursing experience preferred; ambulatory care experience a plus.

Strong knowledge of nursing principles, EMR systems, and patient-centered care models.
