Consumer Directed Community Support (CDCS) Service Coordinator

Description Lifeworks is hiring a full-time Consumer Directed Community Supports (CDCS) Service Coordinator to join our In-Home Services team. In this role, you will provide case management and personalized support to individuals with disabilities and their families. You’ll help clients manage their FMS budgets, navigate Minnesota waiver programs, coordinate services, and ensure their funding aligns with their goals and priorities. As a CDCS Service Coordinator, you’ll work with a high level of autonomy while being part of a collaborative Self-Directed Services team. Travel in the greater metro area when necessary is required. Must pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.). Requirements High School diploma with a minimum of three years direct care experience with disabilities or those aged 65 older or in a related human services field. Bachelor’s or associate degree in human services or related filed with 2 years of relevant work experience preferred. Demonstrated interpersonal skills that quickly build rapport with customers, Lead Agencies, and administrative staff. Case management experience related to self-directed services preferred.
