Job Description

Step into a highly flexible per diem opportunity focused on providing essential healthcare services to patients in their homes. This position involves conducting home health risk assessments for Medicare Advantage plan members throughout Chesterfield County, including areas in and around Richmond such as Midlothian, Chester, and Bon Air. Each assessment typically lasts about 45 minutes and may be scheduled as annual health reviews or post-discharge visits, depending on client needs.

Schedule:

• Flexible shifts from Monday to Sunday
• Design your own schedule
• Must meet a minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage members
• Review and document current/prior medical history, present conditions, and medication usage
• Educate patients and families on health conditions, prescriptions, and self-care practices
• Identify care gaps, offering recommendations for improved patient outcomes
• No requirement for treatment plan creation or medication prescribing

Qualifications:

• Active Nurse Practitioner license in Virginia
• Master's degree in Nursing (MSN) from an accredited institution
• Valid CPR certification (BLS)
• Board Certified through AANP or ANCC
• At least six months of recent Nurse Practitioner experience is preferred
