Physical Therapist

We are seeking a licensed Physical Therapist to provide personalized, one-on-one care to older adults in private homes and senior living communities. Key Responsibilities Include Conducting evaluations, developing individualized treatment plans, and delivering evidence-based therapy to enhance mobility, independence, and quality of life. Documenting patient progress accurately within electronic medical records. Collaborating with an interdisciplinary team of healthcare professionals. Educating patients, caregivers, and staff on safety and aging-in-place strategies. Incorporating telehealth and virtual care to increase flexibility and reach more patients. Qualifications And Skills Required Valid Physical Therapy license and degree from an accredited program. Proficiency with basic computer systems and EMR software. Current CPR certification. The role demands physical effort, including patient transfers, repositioning, walking, and driving to patient locations. Clinicians must be able to lift moderate weights and provide assistance during therapy activities.
