Job Opportunity: Registered Nurse Case Management
Our organization is looking for a dedicated Registered Nurse specializing in Case Management to facilitate safe and effective transitions of care for a varied patient demographic. This position involves a range of responsibilities aimed at enhancing patient outcomes.
Key Responsibilities:- Evaluate both clinical and psychosocial needs of patients.
- Formulate comprehensive discharge plans.
- Collaborate with a multidisciplinary team, including physicians, nurses, social workers, and community organizations.
- Monitor resource utilization and patient progress.
- Provide education and support to patients and their families.
- Advocate for necessary resources to facilitate recovery and minimize readmission rates.
- Current Registered Nurse license.
- Experience in case management or a related field is preferred.
- Strong communication and interpersonal skills.
- Ability to work collaboratively in a team-oriented environment.
