Responsibilities
- Provide direct nursing care to residents following individualized care plans.
- Administer medications and document administrations accurately in the charting system.
- Monitor resident conditions, report changes promptly to the supervising nurse, and respond to emergencies per policy.
- Assist residents with activities of daily living and support functional independence.
- Maintain comprehensive and compliant documentation, including MARs, nursing notes, and assessments.
- Adhere to infection control practices, safety protocols, and care standards; utilize appropriate PPE.
- Collaborate with interdisciplinary team members to coordinate care and ensure resident safety and well-being.

Requirements
- Active Licensed Practical Nurse license in New Hampshire (or ability to obtain)
- LPN diploma or associate degree in Practical Nursing
- Minimum 1 year of recent experience in long-term care or skilled nursing environments preferred
- Current Basic Life Support (CPR) certification
- Ability to work night shifts (3x12-hour nights) and function in a local assignment
