Physical Therapist Home Care (Week-ends)

Overview Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility, relieve pain, and improve or correct disabling conditions resulting from disease or injury. Provides age-appropriate education for clients and caregivers. Supervise and assume responsibility for care provided by identified team members. Location Harrisonburg, VA Work Shift Weekend Days Education Bachelor’s Degree in Physical Therapy (Required) Master’s Degree in Physical Therapy (Preferred) Doctorate in Physical Therapy Certification / Licensure Physical Therapy License (Required) Experience Performs modalities and therapeutic interventions in accordance with accepted standards of care and department policies and procedures. Provides effective instruction to patients and families through standardized educational programs or individualized training. Initiates, develops, and maintains collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes. Works productively and efficiently in daily operations. Selects and performs appropriate assessment procedures. Establishes and updates formal plans of care to address individual patient needs.
