Summary

The Licensed Vocational Nurse (LVN) provides direct nursing services and supports the day-to-day clinical functioning of nursing operations at The Annex, a comprehensive mental health treatment program serving adults across multiple levels of care, including weekends.

Working under the clinical supervision of a Registered Nurse and medical leadership, the LVN plays a vital role in ensuring safe medication administration, accurate clinical documentation, and coordination of medical and psychiatric care within an interdisciplinary treatment environment.

This position contributes to maintaining high standards of client safety, regulatory compliance, and quality care while supporting a collaborative, trauma-informed treatment culture.

Core Responsibilities

• Provide direct nursing care within LVN scope of practice in a behavioral health treatment setting
• Administer medications and treatments as ordered by licensed medical providers
• Monitor clients for medication effectiveness, side effects, withdrawal symptoms, and changes in clinical presentation
• Complete nursing assessments, vitals, and health screenings as assigned
• Accurately document all services, observations, and interventions in the electronic medical record (EMR)
• Support safe medication storage, inventory control, reconciliation, and disposal procedures
• Communicate client medical or psychiatric concerns promptly to RN, providers, and clinical leadership
• Assist with admissions, discharges, and transitions of care
• Coordinate with therapists, case managers, and medical providers to support treatment planning
• Respond appropriately to medical or psychiatric emergencies according to program protocols
• Maintain professional therapeutic boundaries while working with high-acuity clients

Detailed Responsibilities

• Administer and document medications in accordance with physician orders and organizational policy
• Maintain accurate Medication Administration Records (MARs)
• Observe and report behavioral or medical changes to supervising RN or provider
• Support infection control practices and required health and safety standards
• Assist with medication education and reinforcement of treatment recommendations
• Participate in incident reporting and follow-up documentation when medical or safety events occur
• Prepare medications for transport, pharmacy coordination, or provider review as needed
• Assist with laboratory coordination, appointment scheduling, and external medical communication
• Participate in nursing huddles, staff meetings, and quality improvement initiatives
• Maintain cleanliness and organization of medication rooms and clinical supplies
• Support continuity of care across PHP, IOP, and outpatient programming

Required Knowledge, Skills, and Abilities

• Knowledge of behavioral health nursing practices and psychotropic medications
• Ability to work effectively with clients experiencing complex trauma, personality disorders, suicidality, or emotional dysregulation
• Strong observational and clinical reporting skills
• Ability to remain calm and professional in high-acuity or crisis situations
• Excellent communication and interdisciplinary collaboration skills
• Strong attention to detail and documentation accuracy
• Working knowledge of infection control and medication safety standards
• Proficiency with EMR systems and healthcare documentation

Education and Experience

• Graduate of an accredited Vocational Nursing program
• Current, active Texas Licensed Vocational Nurse (LVN) license required
• Minimum 1–3 years of nursing experience preferred; behavioral health or psychiatric experience strongly preferred
• Experience in residential, PHP, IOP, or mental health treatment settings highly desirable
• Current BLS/CPR certification required
• Valid Texas driver’s license required

Schedule & Classification

• May include evenings and weekend coverage based on program needs
