The Palliative Care Nurse Practitioner serves as a clinical leader in the community-based palliative care program. The NP provides advanced assessment, diagnosis, and management of patients with serious illness across care settings. This role emphasizes patient- and family-centered care, expert communication, and coordination with the interdisciplinary team to improve quality of life, align treatment with patient goals, and ensure continuity of care.

ESSENTIAL JOB FUNCTIONS:

• Conduct comprehensive patient assessments, focusing on physical, psychosocial, and spiritual needs.
• Develop and implement individualized care plans in collaboration with patients, families, and the interdisciplinary team.
• Facilitate advanced care planning and goals-of-care discussions with patients, families, and healthcare providers.
• Prescribe medications, treatments, and interventions consistent with licensure and scope of practice.
• Provide clinical leadership and mentoring to the interdisciplinary team.
• Ensure compliance with Joint Commission, state, and federal regulations.
• Participate in quality improvement initiatives and program evaluation.
• Travel required to patient locations throughout the New York metro area.
• Works within legal, ethical practice standards to his/her position as well as Calvary's Code of Conduct. Comply with appropriate policies and procedures and brings potential compliance matters to supervisor or compliance officer.
