Case Manager
We are looking for an experienced RN to join our Case Management team. The ideal candidate will have a minimum of 5 years of experience in nursing. This role requires a valid RN State License and current BLS certification. The position involves working standard hours with a focus on patient care coordination. Responsibilities include managing patient cases and facilitating communication between healthcare providers and patients. Candidates will need to complete various health and human resources forms upon hiring. All necessary health screenings and vaccinations must be in compliance with requirements. The role also includes participating in skills assessments and competency validations. Submissions must include required certifications and licensure documents.
