Summary

We are seeking a dedicated and highly organized Case Manager to coordinate and monitor services and supports for individuals residing in our Intermediate Care Facility for Individuals with Intellectual Disabilities (ICF/IID).

Reporting to the Case Management Supervisor, you will serve as an advocate and key member of the interdisciplinary team, facilitating person-centered planning, developing and monitoring Individual Program Plans and Behavior Support Plans, coordinating services, monitoring participant progress, and ensuring compliance with applicable ICF/IID regulations and organizational standards.

The Role

• Meet regularly with participants to understand their needs, preferences, strengths, goals, and desired outcomes

• Advocate for services and supports that promote participant independence, choice, dignity, health, safety, and quality of life

• Develop, implement, review, and update Individual Program Plans (IPPs) in accordance with ICF/IID requirements

• Ensure services and supports are person-centered and responsive to changing participant needs

• Coordinate and monitor medical, behavioral, residential, vocational, recreational, community-based, and other services identified through each participant's plan

• Facilitate and lead annual IPP and interdisciplinary team meetings, ensuring participation from the participant, guardian or family, and appropriate team members

• Document recommendations, decisions, action items, and required follow-up from interdisciplinary meetings

• Monitor participant progress and changing needs and initiate revisions to services, supports, and plans as appropriate

• Complete required monthly progress notes and other regulatory documentation accurately and within established timeframes

• Serve as a primary point of contact for participants, families, guardians, internal staff, and outside service providers

• Respond appropriately to emergency and behavioral situations

• Track and report behavioral incidents in accordance with state and organizational requirements

• Develop, write, implement, and monitor Behavior Support Plans in collaboration with the interdisciplinary team

• Accompany participants to medical appointments, counseling sessions, community activities, and other services when needed

• Train direct care staff on participant goals, support plans, and required daily documentation

• Collaborate with nursing staff, therapists, residential teams, and other professionals to coordinate ongoing care

• Maintain a caseload of approximately 16 participants with varying ages, needs, and levels of support

• Support participants in engaging meaningfully in campus and community activities based on their interests, preferences, and individual needs

• Assist participants with money management and understanding insurance benefits as appropriate

• Support activities that help participants develop independence and stronger connections to their community

• Help ensure participants can safely access buildings, programs, and community activities based on their physical needs

• Balance participant choice and independence with assessed risk, health, safety, and regulatory requirements

• Maintain accurate, confidential records and comply with all applicable privacy requirements

• Travel locally, approximately 10%, for community activities, appointments, and participant support needs

Qualifications

• Bachelor's degree in social work, psychology, healthcare, or a related field

• Minimum of one year of experience working with individuals with developmental disabilities

• Previous experience working in an ICF/IID setting strongly preferred

• Comprehensive knowledge of ICF/IID regulations, standards, documentation requirements, and person-centered practices preferred

• Qualified Intellectual Disabilities Professional (QIDP) certification or ability to complete certification within six months of hire

• Willingness to complete ongoing training required to maintain QIDP qualifications

• Valid driver's license with a clean driving record

• Ability to complete CPR, First Aid, and organizational crisis intervention training

• Ability to complete the Direct Support Professional training program within the first year of employment

• Knowledge of applicable state and federal requirements, including DHS, IDPH, HIPAA, and Medicaid regulations

• Strong written and verbal communication skills in English

• Strong organizational, documentation, and follow-through skills

• Ability to effectively coordinate with participants, families, guardians, healthcare providers, staff, and external service partners

• Sound judgment and problem-solving skills when responding to changing participant needs and crisis situations

• Strong advocacy skills with a commitment to participant rights, dignity, choice, and independence

• Ability to work collaboratively as part of an interdisciplinary team

• Computer proficiency, including word processing, spreadsheets, database systems, email, and digital communication platforms

• Ability to manage multiple priorities while maintaining accurate and timely documentation

• Cultural sensitivity and ability to work respectfully with individuals from diverse backgrounds

• Ability to maintain confidentiality and professional standards in accordance with organizational and regulatory requirements

• Ability to walk, stand, and sit for extended periods throughout the workday

• Ability to lift, carry, or move mobility aids and support equipment weighing up to 50 pounds

• Ability to provide direct physical assistance with activities of daily living, including medication administration or medication passes, toileting, bathing or showering, personal hygiene, dressing, and other personal care needs

• Ability to bend, reach, lift, and physically support individuals with varying levels of assistance needs

• Ability to drive personal or organizational vehicles for participant transportation and community activities

• Flexibility to provide occasional evening, weekend, or holiday coverage based on participant needs and emergency situations

The starting hourly pay for this role is $26.00 /hr.
