Job Details Job Location: Brooks Rehabilitation Hospital-Arizona - Phoenix, AZ 85054 Position Type: Full Time Job Shift: Any Job Category: Case Management Reporting to the Manager of Case Management, the Case Manager Assistant supports the case management team in coordinating a smooth and timely discharge process. This role facilitates discharge services, appointments, DME needs, and access to community resources. The ideal candidate is organized, communicative, compassionate, and able to problem-solve effectively. Job Responsibilities Coordinate post-discharge services, appointments, and required documentation. Collaborate with DME providers to ensure equipment is available at discharge. Communicate effectively with patients and families regarding discharge needs. Assist with connecting patients to community resources and support services. Support team conference documentation and compliance requirements. Maintain accurate and timely records. Promote a positive patient experience through compassionate communication and service. Job Qualifications High school diploma or equivalent required. One year of healthcare experience preferred. Strong verbal and written communication skills. Computer proficiency. Excellent organizational and time-management skills. Strong problem-solving and follow-through skills.
