In-Home Nurse Practitioner - Part-Time Health Evaluator
NPACE
Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP or PA) to perform in-home health evaluations for members. The visits last about 45 minutes and focus on health questions, medication review, and brief physical assessments in the patient’s home. You will travel locally and within licensed states, use an iPad with a clinical workflow, and may conduct virtual visits as needed. A current, unrestricted license, board certification, and multi-state credentials are required or #J-18808-Ljbffr
