Hospital Social Work Case Manager — Care Coordination & Discharge

Allegheny Health Network seeks a social work professional to collaborate with an interdisciplinary team to coordinate care across the continuum. The role focuses on social determinants of health to improve patient outcomes and experience within a hospital setting. Required are a Master’s in Social Work and PA LSW/LCSW licensure (or within the permitted timeframe). Clearances (Act 34/33/73) are required.
