Overview About Primecare Hospice & Pallative Care At Primecare Hospice, caring is not just our duty; it is our purpose. We are a patient-centered, physician-led, and team-based holistic hospice program. We are dedicated to making every moment matter for patients and their families with empathy, respect, and unwavering commitment. Hospice is primarily a home care program, but services may also be provided in a hospital or nursing home. The hospice team, called the interdisciplinary group (IDG), helps families make informed decisions about caregiving, teaches skills for care, and supports end-of-life care. Bereavement support is available after the death of a loved one. Position Summary To plan, coordinate and provide hospice care in accordance with the patient’s plan of care. Essential Duties Coordinate hospice care by working with other team members and with the patient’s/family’s needs. Evaluate the effectiveness of hospice services for the patient and family on an ongoing basis. Perform evaluations for hospice care and conduct informational visits as needed. Utilize the nursing process to provide professional nursing care. Consult with the attending physician and Hospice Medical Director regarding changes in patient condition and plan of care. Coordinate patient services with IDG members and contracted services when needed. Submit complete, accurate, and timely clinical notes and patient visit schedules. Participate in IDG and case conferences. Engage in ongoing professional development and education to improve nursing care skills. Educate patients and family members regarding the dying process, plan of care and healthcare decisions. Provide dietary counseling considering health issues and end-of-life dietary changes. Conduct death visits per agency policy. Participate in discharge planning. Maintain current knowledge of pharmacotherapy. Adhere to Federal, state, and accreditation requirements including Medicare and Medicaid regulations. May be requested to fill in for other nurses as needed. Conduct initial and ongoing comprehensive patient assessments, obtain medical histories related to current condition, and perform physical examinations including vital signs, mental status, appetite, and diet. Evaluate the home environment and caregiver capabilities to determine necessary health teaching and services. Determine appropriate levels of care and communicate nursing and Agency services to patients and families. Develop and implement the initial plan of care with IDG collaboration and maintain the comprehensive care plan. Provide nursing treatments and procedures ordered by the physician and monitor patient responses. Supervise and evaluate the care provided by the Hospice Aide at regular intervals and as needed. Document services rendered, chart revisions, supervisory visits, and progress notes; manage charts for discharged patients. Provide guidance to LPNs and supervise them on a regular basis. Collaborate with other agencies to ensure continuity of care and a comprehensive care plan. Participate in staff development meetings and prepare the care plan for the Hospice Aide. Required Knowledge, Skills, Abilities and Competencies Available to be on call. Demonstrates self-confidence, positive attitude, and commitment to goal achievement. Experience and professional relationships aligned with the organization’s mission. Ability to develop and implement improved methods of operation. Ability to identify and evaluate personal strengths and weaknesses in self and others. Ability to supervise objectively without personal bias. One year of RN experience; hospice experience preferred. Knowledge of applicable local, state, and federal laws. Excellent assessment, problem-solving, and communication skills. Must be a licensed driver with an insured automobile in good working order. Knowledge of spreadsheets. Excellent written and verbal communication; strong organizational and time-management skills. Ability to exercise discretion and professionalism when handling confidential information. RN license without exceptions and the ability to administer medications, perform medication reconciliation, and other nursing procedures as permitted by state practice; competencies to be demonstrated before hands-on care. Salary & Benefits Annual Salary Range: $80,000 – $90,000 On-Call Pay Structure: $150 per weekday on-call (Monday – Thursday); $450 for weekend on-call (Friday – Sunday) Work Schedule Standard Hours: Monday – Friday, 8:00 AM to 5:00 PM On-Call Duties: Weekends and As Needed #J-18808-Ljbffr Flournoy Health Systems
