Maintains competencies within practical nursing standards and seeks opportunities to develop professionally. Assumes responsibility for self-development in lifelong learning. Communicates effectively and works cooperatively with others while showing respect for and an understanding of other clinical disciplines. Provides care to patients whose care requirements are routine/standardized and with predictable outcomes./ppJob responsibilities:/pulliFollows the seven rights of medication administration while reporting adverse reactions to the physician immediately and taking action in emergency cases./liliCoordinates referrals for patient chronic care conditions and follow-up care as indicated by the initial care plan of the specialist and/or primary care physician./liliAdministers prescribed oral and intramuscular medications, therapies, and treatments following practical nursing standards and under the direct supervision of the physician./liliEffectively communicates with healthcare team members, patients, and families; supports healthcare team members to achieve team goals and assigns appropriate tasks to care assistants./liliMaintains and reviews patient medical records, notes, and physician orders; documents all patient care activities in accordance with documentation standards and legal requirements for medical facilities./liliUpholds the standards of Patient-Centered Medical Home (PCMH) and assists other clinical staff members in completing and following quality measures under PCMH and Patient-Centered Specialty Practice (PCSP)./liliCreates a plan of care that is patient-centered and provides a written copy of the plan of care to patients./liliProvides direct patient care, utilizing the nursing process, under the direction and supervision of the physician./liliMay perform additional duties as assigned./li/ulpRequirements:/ppCertification - Basic Life Support/ppLicensure - Licensed Practical Nurse licensed or eligible for licensing in the state of employment as a Licensed Practical Nurse/ppExperience - 2 years of nursing practice in a hospital or clinic setting/ppEducation - Vocational/Technical Graduate of an accredited Licensed Practical Nurse school/ppKSAs - Technology use; Effective relationships/Interpersonal skills; Collaboration; Effective communication/ppPreferred experience: clinical experience in outpatient medical office setting/ph2About Us/h2pWe are Inova, Northern Virginias leading nonprofit healthcare provider.
