Monitor residents’ conditions, document vital signs, and report changes to the supervising RN. Assist residents with activities of daily living, promoting dignity, safety, and mobility. Perform routine wound care, dressing changes, and other standard nursing tasks as directed. Document care provided and resident responses in the resident chart or EMR; communicate with the care team to support care planning.
