RN - Hospice

RN Care Manager

The position involves managing patient care in a hospice setting. The RN Care Manager will provide comfort, compassion, and quality care to patients and families during critical times. Substantial, specialized bedside nursing skills are required for effective symptom management, crisis intervention, and family education. Excellent written and verbal communication skills are essential for this role. The candidate should possess strong problem-solving and decision-making abilities. Experience in developing and maintaining rapport with patients and families is necessary. The ability to work collaboratively with an interdisciplinary team is important. The role primarily involves working in patients' residences. Travel to long-term care facilities, hospitals, and area agencies will be necessary. Travel within the designated service area will be required. Overnight travel for continuing education may also be necessary. Employees are at direct risk for exposure to infections and blood-borne pathogens.
