Compassionate Hospice Intake & Care Liaison

Gentiva, Heartland Hospice, seeks an experienced Hospice Clinical Liaison to ensure efficient intake and referrals. You will build strong relationships with hospitals, discharge planners, physicians, and families, coordinating admissions and advocating for patients’ needs.

You will travel to hospitals and facilities, educate stakeholders about hospice, and help expand community awareness. A nursing background with hospice experience is required, with BSN preferred, CPR certified, and a valid
