Job Details Job Location: 3800 Florin Road - SACRAMENTO, CA 95823 Salary Range: $26.00 - $28.00 Hourly *Role may need to float to other SNAHC locations Position Summary The Community Health (CH) Program Coordinator reports to the CH Program Manager. This is an integral position to the operations of grant programs focused on planning, developing, improving, and delivering mental health prevention activities to Native American/Indigenous youth and their families. Working collaboratively with the CH Program Manager, the Program Coordinator will be responsible for leading, coordinating and ensuring execution of required activities for community health programming. This position will receive training and support from the CH Department and contracted evaluation team to fulfill these responsibilities. Essential Functions Supports the establishment and implementation of age-appropriate, culturally responsive programs, groups, training activities, health education classes, and interventions related to substance use and/or suicide prevention. With guidance from the Program Manager, coordinates and implements grant funded prevention and/or early intervention services and activities. Maintains accurate and timely documentation of services and program participation, including digital photographs of programming, and requests for marketing, and/or outreach. Assists with grant applications, reporting, and evaluation activities. Assists in partnerships that engage key stakeholders and community members in addressing community health priorities and meeting grant deliverables. Supports planning for and onsite staffing of SNAHC Gathering of Native Americans (GONA) and other community health events. Plans, prepares for, and participates in events such as health fairs, pow wows, conferences, and trainings. Coordinates, sets up and breaks down outreach booths at community health events and other outreach opportunities. Ensures SNAHC services and programs are delivered in a culturally appropriate manner. Participate in internal quality improvement teams. Works with team members to actively participate in quality improvement initiatives in accordance with the mission and strategic goals for the organization, federal and state laws and regulations, and accreditation standards. Complies with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc. Other duties as assigned. Skills and Abilities Desire to work closely with youth and families. Familiarity with client-driven care. Ability to work both independently and as part of a team. Excellent verbal and written communication skills. Must have excellent organizational and computer skills including experience with word processing and spreadsheet skills. Competencies Communication and Relationships Initiative Planning and Organizing Safety Teamwork Customer Service Coordination/Event Planning Data Collection Minimum Qualifications Experience coordinating, implementing and/or monitoring community and/or grant funded programs. Must be able to lift 40 lbs and be responsible for the set up and breakdown of outreach booth and equipment Must have a valid California Driver’s License with current auto insurance and reliable transportation. Must be able to work nights and weekends as needed Preferred Qualifications 2 years of experience coordinating, implementing and/or monitoring community and/or grant funded programs. 2 years of experience in direct service in youth prevention. Experience in an FQHC setting as a member of a multi-disciplinary team preferred. Knowledge of traditional, cultural and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.
