About the Position

Job Title: Case Manager - RN Profession: Registered Nurse Specialty: Case Management Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum of 4 years of experience as a Registered Nurse is required. Two years of health care experience is also required. Epic experience is required. License: Must be licensed to practice as a Registered Nurse in the state of North Carolina. Certifications: BLS is required. ACM/CCM is preferred. Must-Have: No first-time travelers are eligible. Candidates must be located outside a 50-mile radius from the facility. Description: The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction. This includes promoting continuity of care and cost-effectiveness through integrated functions of case management, utilization review, and discharge planning. The Case Manager must be a highly organized professional with great attention to detail. The individual must be adaptable to frequent change and compliant with regulatory and departmental guidelines and policies.
