Available for all HMC cost centers: East - 1114, West - 1115, ACT - 12026, Nursing Home - 12027, Cates - 13016, Brandley - 13017, and Hall - 13018."

Duties:

Job Summary: Our client is seeking highly motivated and compassionate Home Health PT Physical Therapists fulltime for our [insert cities/county] Team. As part of our Physical Therapy Team, you will be part of a dynamic Clinical Team who ensure that our patients return and maintain their optimum level of physical functioning. Job Responsibilities: -Provide physical therapy services to patients as prescribed by a physician in compliance with agency policies and procedures and federal and state regulations. -Perform initial and ongoing assessments, including OASIS assessments at appropriate time points, to determine level of functioning and patient's ability to perform activities of daily living. -Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patients to achieve realistic goals of mobility. -Reports to the physician and designated agency representative patient's reaction to treatment and any changes in the patient's condition.

Skills:

Required Skills & Experience: -One to two (1-2) years of experience. Preferred Skills & Experience: -Minimum of one (1) year of experience in home health care.

Education:

Required Education: -DPT from CAPTE accredited program. Preferred Education: -N/A Required Certifications & Licensure: -Licensure as a physical therapist in the state of Florida. -BLS from the American Red Cross or American Heart Association. Preferred Certifications & Licensure: -N/A
