Field-Based RN Case Manager - Wound Care & Infusion

Adaptive Home Health is seeking an RN Case Manager to deliver high-acuity home health nursing across Texas. You will own assessments, manage wound care and infusions, and coordinate a comprehensive plan of care with physicians and families.
You will advocate for patients, ensure continuity of care, and drive timely follow-ups while supporting LVNs and HHAs. A strong clinical background, autonomy, and compassionate patient care are essential.
