American Traveler is seeking a Physical Therapist for a 13-week home health assignment in a rural, multi-county service area, requiring at least 1 year of experience and active BLS certification.
Details- Home health care setting serving four counties (Noble, Lagrange, Steuben, and DeKalb), primarily rural with mid-sized towns
- Caseload equivalent to approximately 4–6 patient visits per day using a point-based tracking system
- Common cases include elderly falls and fractures
- Responsibilities include Oasis Assessments and Start of Care cases
- Charting system: Homecare Homebase (HCHB)
- Tablet used for time and visit tracking
- Schedule: Monday–Friday, 8:00 AM–5:00 PM; day shift, 8-hour shifts
- Minimum 1 year of physical therapy experience required
- Active PT state license or Compact (NLC) licensure required; pending licenses will not be accepted — license required if applicable
- Active BLS (AHA) certification required; pending certifications will not be accepted
- Valid driver's license required for consideration
- 2 professional references required: one supervisor from within the last 12 months worked, and one peer or supervisor from within the last 3 years worked, including dates of employment and eligibility for rehire
- 12-hour orientation provided
- Attire: business casual or hunter green scrubs with a white or black top
- 13-week assignment with a standard 13-week cancellation policy
