SNF Wellness CNA Location: PRN Manual Labor San Antonio, TX, US General Summary The Wellness CNA assures the day-to-day care of residents and patients in a sensitive, professional, and effective manner, following state and company regulations. The role may also participate in the Culinary Program and Community Program to ensure the best quality of care. Essential Job Functions Assist residents/patients with personal care and activities of daily living as assigned and requested. Review and participate in 24‑hour reports, daily logs, shift cross‑over reports and verbal reports. Assist residents/patients to and from scheduled programs. Promote personal choices, independence and spiritual needs. Perform laundry duties as assigned. Respond to resident/patient emergency calls promptly. Ensure compliance with new resident/patient care protocols. Report all resident changes in condition immediately to the Wellness Nurse or Resident Wellness Director. Maintain infection control practices. Report all accidents and incidents as soon as possible to supervisor. Maintain confidentiality of residents’/patients’ personal care information. Honor residents’/patients’ personal and property rights. Be prepared for emergency situations including disasters, fire, and other emergencies. Perform daily housekeeping tasks using established procedures. Assist in the dining room with serving meals and clearing tables. Provide post‑mortem care as instructed. Always wear company uniform and be resident ready. Perform other related essential duties as assigned. Knowledge, Skills, and Abilities Communicate effectively with all levels of management, team members, residents, family members, vendors, referral sources, and outside contacts. Make independent decisions when appropriate. Communicate warmly, friendly and caring. Show passion for working with and around senior citizens. Understand customer service principles and practices. Education Requirements High School Diploma or equivalent. Certified Nursing Assistant. CPR and First Aid certification. Experience Requirements Experience working with seniors preferred. Physical Requirements Stand or walk 75% of the day. Concentrate with frequent interruptions. Work under stress and in emergency situations. Perform activities requiring sitting, standing, walking, reaching, pulling, pushing and grasping. Talk and hear effectively. Occasionally lift or carry up to 50 pounds. Occupational Exposure / Personal Protective Equipment Work throughout the entire community. May be exposed to dust, disinfectants, cleaning chemicals, infectious waste, diseases, substances, and odors. Use personal protective equipment and supplies to prevent burns, falls, and infection, following safety policies and procedures. Required protective gear includes an infection control kit, rubber gloves, and non‑slip shoes. Hours & Compensation Shift schedule: 6 am–2 pm, 2 pm–10 pm, 10 pm–6 am. Schedule as needed.
