Responsibilities Conduct medication reviews and assessments to ensure optimal therapeutic outcomes. Provide clinical consultation to healthcare teams and nursing staff on medication-related issues. Develop and implement individualized pharmaceutical care plans for patients. Monitor patient medication regimens for safety and efficacy, adjusting as necessary. Educate patients and staff on medication management and best practices. Stay current on pharmacy regulations and industry trends to ensure compliance. Collaborate with interdisciplinary teams to improve overall patient health outcomes. Participate in quality assurance initiatives and pharmacy audits. Requirements PharmD degree from an accredited pharmacy school. Active pharmacist license in California. Minimum of 2 years of experience in a clinical pharmacy setting. Strong knowledge of pharmacotherapy and medication management. Excellent communication and interpersonal skills. Ability to work collaboratively in a fast-paced environment.
