This position is entirely remote, allowing for flexible work arrangements. While you will be working from a climate-controlled home office setting, interaction with colleagues and other healthcare providers will be encouraged through virtual meetings and collaborative tools. Your workday will involve digital communication with clients, families, and a multidisciplinary care team dedicated to achieving optimal patient outcomes.

Key Responsibilities\n

  • Conduct comprehensive assessments and diagnoses, as necessary, of patients' mental health needs using appropriate tools, including the Diagnostic and Statistical Manual of Mental Disorders (DSM-V), to formulate individualized care plans.

  • Continuously monitor and evaluate the effectiveness of mental health interventions, adjusting treatment strategies to best support patient progress.

  • Maintain accurate and timely documentation of treatment records, progress notes, and reports, adhering to regulatory standards and departmental protocols.

  • Engage collaboratively with patients and families to co-create personalized treatment plans that align with their unique goals and cultural considerations.

  • Implement therapeutic interventions aimed at promoting patient safety and symptom reduction, working within a team-focused environment to achieve shared objectives.

  • Coordinate referrals to additional services and ensure clear communication between patients and the broader treatment network.

  • Mentor and educate students, interns, and volunteers, contributing to the nurturing of future professionals in the field.

  • Participate in peer reviews, clinical collaborations, and chart audits to support the ongoing development of team competencies.

Patient Population / Client Focus\n

The primary focus of this role is providing support to individuals and families experiencing the complex emotional and psychological challenges associated with cancer diagnoses. As a member of our hospice care team, you will play a vital role in addressing the unique needs of this vulnerable population, helping them navigate their mental health challenges in a compassionate, empathetic manner.

Qualifications\n

  • A Master’s degree in Social Work from an accredited institution.

  • Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of Virginia.

Compensation and Benefits\n

The position offers a competitive salary ranging from USD 90,000 to 105,000 annually, reflecting experience and qualifications.
