Work Environment

The work setting is primarily onsite through home visits, allowing you to build meaningful connections with your patients in familiar surroundings. You will work closely with a multidisciplinary team to address patient needs effectively. Key Responsibilities

• Conduct comprehensive evaluations, encompassing a review of medical history and performance assessments, determining tailored treatment plans based on findings.
• Articulate evaluation results and treatment strategies clearly to patients and their families, actively engaging them in setting achievable rehabilitation goals.
• Implement individualized treatment protocols that reflect clinical assessments and adapt them as necessary based on patient progress and evolving healthcare standards.
• Collaborate with other healthcare professionals to ensure comprehensive patient care, documenting any changes in condition that necessitate modifications to treatment plans.
• Manage discharge planning and follow-up care, meticulously documenting patient interactions, progress, and final evaluations to create a clear continuum of care.
• Exhibit proficient time management by maintaining an accurate schedule, arriving punctually, and completing necessary documentation in a timely manner.
• Contribute to departmental goals and objectives while adhering to regulatory and accreditation standards.
• Engage in continuing education initiatives, pursuing professional growth opportunities and serving as a mentor to students and new team members.
• Foster a supportive patient experience by demonstrating flexibility and adaptability in care delivery.
• Participate in community outreach efforts, advocating for improved access to therapy services and understanding patient demographics through Continuous Service Improvement (CSI) initiatives.

Patient Population / Client Focus

Your primary focus will be on patients receiving home care services, ranging from post-surgery rehabilitation to ongoing support for chronic conditions. You will work with diverse patient populations, including elderly individuals, stroke survivors, and those recovering from orthopedic surgeries, ensuring that each person receives a personalized plan that considers their home environment and personal goals.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid DPT license in the State of New Jersey is required.
• Current American Heart Association BLS certification for Healthcare Providers.
• Commitment to maintaining professional licensure and compliance with state regulations.

Preferred Experience

• A minimum of 2 years of experience in physical therapy, particularly in home health care or outpatient settings, is preferred but not required.
• Experience utilizing evidence-based practices to inform treatment decisions.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually , commensurate with experience. We offer a comprehensive benefits package that includes:
