Occupational Therapist, Home Health

CenterWell Home Health is seeking a compassionate and skilled Occupational Therapist, Home Health, to provide personalized, in-home therapeutic services to adult and geriatric patients. In this role, you will travel to patients’ residences to assess functional abilities, develop and implement individualized treatment plans, and promote independence in activities of daily living within the home environment. You will collaborate closely with an interdisciplinary healthcare team, including nurses, physical therapists, speech therapists, physicians, and caregivers, to ensure coordinated, high-quality care that supports patient safety, autonomy, and quality of life. Responsibilities include conducting comprehensive evaluations, setting measurable goals, providing hands-on therapeutic interventions, recommending adaptive equipment and home modifications, and educating patients and family members on safe techniques and strategies for daily tasks. You will accurately document assessments, progress notes, and care plans in accordance with regulatory and CenterWell standards, while adhering to all infection control and patient safety protocols.
