We are seeking a compassionate and skilled Occupational Therapist to join our therapy team at Brookwood Gardens Rehabilitiation and Nursing Center, a Skilled Nursing Facility dedicated to providing exceptional care to our patients. As a valued member of our team, you will play a crucial role in helping our residents achieve their highest level of physical, emotional, and cognitive function.
Responsibilities:
Develop and implement individualized occupational therapy plans to meet the unique needs of our residents
Conduct evaluations and assessments to identify areas of strength and weakness
Design and implement tailored interventions to improve functional abilities, such as mobility, ADLs, and cognitive function
Collaborate with other healthcare professionals to ensure comprehensive care and achieve optimal outcomes
Maintain accurate and timely records of patient progress and therapy sessions
Participate in interdisciplinary team meetings to discuss patient care and develop effective treatment plans

Requirements:
Bachelor's degree in Occupational Therapy or related field
