Synergy Care is seeking a full-time Certified Occupational Therapy Assistant (COTA) for an outpatient clinic serving an Assisted Living/Independent Living (AL/IL) community at Franklin Park in Boerne, TX .
Join our team! We are looking for a compassionate, collaborative clinician who is dedicated to providing exceptional patient care and making a positive difference in the lives of those we serve.
✨ Position Responsibilities- Implement treatment plans established by the Occupational Therapist
- Provide skilled occupational therapy treatments to residents
- Document patient progress and communicate with the therapy team
- Assist with discharge planning and patient education
- Participate in interdisciplinary case management
- Deliver high-quality therapy services in accordance with state regulations and Synergy Care standards
- Represent Synergy Care's commitment to excellence in every patient and facility interaction

- Graduate of an ACOTE-accredited Occupational Therapy Assistant program (or graduating within 6 months)
- Current NBCOT certification (COTA) required
