You will implement fun, creative, and personalized activities , helping participants reach their personal best at home, at work, and in the community. Your role includes providing guidance in daily living and supporting community engagement.

Key Responsibilities:

Daily Housing Support & Community Engagement:

• Implement individualized, engaging activities tailored to each participant
• Support participants in reaching their personal goals and maximizing independence
• Provide guidance and encouragement in home and community settings
• Foster community inclusion and involvement
• Support individuals in their daily routines such as cooking, cleaning, laundry, transportation, recreation and personal hygiene

Relationship Building & Enrichment:

• Utilize your own talents, skills, hobbies, and interests-such as art, music, cooking, sports, or other personal passions-to enrich the lives of participants

Minimum Requirements:

• Must be at least 21 years old
• High School Diploma/GED
• One year of experience in Human Services (preferred)
• Active, clean, non-restricted Connecticut Driver's License (minimum 3 years U.S. possession)
• Clean driving history
• Reliable transportation
• Ability to safely lift at least 75 lbs.
• Ability to stand, walk, and sit for periods at a time.

Physical & Work Environment Requirements:

• Work occurs in home and community settings
• Requires physical activity including lifting, standing, and walking
