Justice‑Involved Case Manager (Reentry & Prison Coordination) Location: Arizona – Statewide (priority hiring near correctional facilities and surrounding zip codes) Field-Based + Community + Court + Facility Coordination Position SummaryWe are seeking motivated, mission-driven Case Managers to support justice‑involved individuals preparing for release and successful reentry into the community.This role focuses on serving individuals connected to Arizona correctional facilities, probation/parole systems, and reentry programs, ensuring continuity of care, compliance with court/program requirements, and long-term stability.The ideal candidate understands that reentry is not just about release-it's about structure, accountability, and support across multiple systems.This position plays a direct role in:Reducing recidivismImproving treatment engagementStabilizing housing, employment, and recovery outcomesThis aligns with your internal program model where case managers provide care coordination, service linkage, and compliance support for justice‑involved populations. Key Responsibilities Justice System CoordinationMaintain communication with correctional facilities, probation, parole, and courts Support individuals in understanding and complying with legal obligations Coordinate reentry services prior to and immediately following release Case Management & Care CoordinationConduct assessments, identify needs, and develop individualized service plans Connect clients to:HousingEmploymentBehavioral health and SUD treatmentMedical and community resources Provide ongoing follow-up and progress monitoring These responsibilities align with your structured case management model including assessment, planning, coordination, and advocacy for justice‑involved clients Reentry & Stability Support Assist clients transitioning from incarceration into the community Support routines, accountability plans, and service engagement Address barriers (transportation, documentation, benefits, etc.) Documentation & ComplianceMaintain accurate, timely documentation Ensure compliance with HIPAA, 42 CFR Part 2, ADHS, and AHCCCS standards Participate in case staffing, team meetings, and reporting Community & Facility EngagementTravel within assigned regions near correctional facilities Build relationships with reentry coordinators and facility staff Represent the organization in justice-system collaboration efforts Required Qualifications Bachelor's degree in:Social WorkBehavioral HealthCriminal JusticePsychologyRelated field OR equivalent experience Experience working with:Justice‑involved individualsBehavioral health and/or substance use populations Strong knowledge of:Court systems, probation, and parole processesCommunity resources and reentry services Ability to:Maintain professional boundariesWork independently in field-based environmentsManage documentation and multiple caseloadsValid Arizona driver's license & reliable transportationAbility to pass background checks and fingerprint clearance Preferred Qualifications Experience inside or working with:Prisons, jails, or reentry programsProbation or parole populations Bilingual (English/Spanish) Training in:Trauma-informed careMotivational InterviewingSolution-focused practices Work EnvironmentCommunity-based, facility-based, and office-based workTravel required within assigned Arizona regionRegular interaction with justice system partners and high-need populations Why Join UsDirect impact on reducing recidivism and improving community outcomesOpportunity to build partnerships with correctional systems across ArizonaMission-driven, growth-focused behavioral healthpostorganization As part of our commitment to maintaining a safe and productive work environment, Sanctuary Recovery Centers conducts background checks and drug screenings for all potential employees.
