Employment Type: Full time Job Summary A Hospice Medical Social Worker provides compassionate, patient‑centered support in patients’ homes, helping families navigate emotional, social and practical challenges of illness and recovery. Responsibilities Provide compassionate, patient‑centered support in patients’ homes. Assess psychosocial needs and connect patients with community resources. Support care planning and advocate for goals that promote dignity, stability and independence. Minimum Qualifications Master's Degree from an accredited school of social work (Council of Social Work Education). Valid State of Michigan Master Social Work License. Minimum two (2) years of social work experience in a healthcare setting. Ability to serve acutely and chronically ill patients.
