Care Manager, Hospice RN

We are seeking a dedicated Registered Nurse to deliver personalized, one-on-one care in the comforting setting of patients' homes. Your responsibilities include providing compassionate end-of-life care to ensure the physical and emotional well-being of patients and their families. You will deliver expert pain and symptom management, offering comfort and support during challenging times. Collaboration with an interdisciplinary team, local physicians, and community providers will be key to optimizing the quality of life for patients. This position requires a rapid response for crisis situations. Holiday coverage is required.
