Hospice Admissions & Referral Liaison (RN)

Gentiva Hospice is seeking an experienced Hospice Clinical Liaison to ensure intake and referral processes are completed efficiently, coordinating admissions and communicating with hospitals, physicians, and families. Responsibilities include evaluating patient eligibility, educating families about hospice services, and building strong referral relationships.
A current RN license is required with 3–5 years of hospice experience; BSN preferred.
