Title: Nurse Practitioner, Advanced Practice Provider Company: Oak Street Health Location: 11515 Sutphin Blvd, Jamaica, NY 11434 The purpose of a Nurse Practitioner at Oak Street Health is to provide effective and equitable value‑based primary care to adults on Medicare, ensuring they remain healthy and avoid hospitalization. Nurse Practitioners see patients independently, collaborate with physicians, and provide longitudinal care that fosters trust and strong patient relationships.
