Registered Nurse (RN) in ICU

The RN implements professional nursing practice centered on the patient as a whole person. A caring environment focused on healing and wellness is created. Nursing practices are based on evidence, current research, hospital policies, and regulatory standards. Clinical decision-making incorporates all phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through collaborative relationships among nursing staff and across disciplines. Direct patient care activities on the unit include nursing rounds, assessments, and treatments. The RN facilitates admission, transfer, and discharge activities. Patient and family-centered education is provided. Care coordination and individualized treatment planning are essential components of care. Responsibilities include: Performing and documenting comprehensive assessments in accordance with scope of practice and policies. Considering the patient's culture, age, and developmental level during assessments. Identifying patient risk factors and presenting complaints. Collecting objective and subjective data related to patient health. Verifying initial admission assessments performed by other healthcare team members. Developing a problem list and prioritizing patient needs with input from the inter-professional team. Designing actions and interventions relevant to patient culture and age. Determining desired outcomes in coordination with the patient and family. Implementing patient care therapies and procedures according to established policies. Communicating procedures and treatments to patients and families effectively. Recognizing and responding to changes in patient condition. Conducting ongoing assessments and documenting responses in accordance with established standards. Delegating tasks appropriately and overseeing team members' performance. Monitoring the accuracy and completeness of delegated patient care. Identifying and addressing educational needs of patients based on comprehension levels. Facilitating the discharge process and coordinating plans with appropriate team members. Providing input into patient/nurse assignments based on staffing and patient acuity. Actively participating in unit-based activities to support organizational values. Promoting a positive group dynamic through words and actions. Assuring nursing excellence in accordance with accepted standards and evidence-based practices. Participating in quality improvement activities and core measure audits. Using a holistic perspective regarding patient care.
